What to Bring to Your Appointment
Please bring your child’s social security card, a valid insurance card and your co-payment (if applicable) to each appointment. Also, please bring your child's immunization record to any well check appointment. If you have new insurance and have not yet received your insurance card, you will need the following information: insurance company name, claim address, co-payment amount, and policy holder name, policy number and group number.

Click here for a list from our WIC program of what to bring as proof of income.

Telephone
Feel free to call us during office hours. Many problems may be solved with advice over the phone, saving you the time and cost of a visit. We are happy to provide this service, but it is against the policy of this office to treat by phone when there is a more serious illness or has never been seen in our office. Whenever the office is closed, one of our providers can always be reached by calling our office number, (315)782.6400. Please call at any hour if your child is truly ill but save routine calls for regular office hours.

If you have Caller ID, it is likely that your phone will not accept blocked calls (those that come up on caller ID as "Private" or "Out of Area"). If this is the case, you must turn off anonymous call blocking in order to receive a call back. Ask our answering service for instructions if you do not know how to do this. If you do not turn this function off, you may not get a call back.

Well Checks
We encourage you to make summer well check appointments well in advance for camp, school, or sports physicals. We are very busy with these appointments in July and August and may not be able to accommodate you on short notice during this time of year. Please bring your child's immunization record to all well check appointments and appointments for school, camp and sports physicals, Head Start physicals and the like.

Immunization Records and Physical Forms
There is a 48 hour turnaround time for these forms to be filled out unless you bring them with you at the time of your well check/physical appointment.

Prescriptions
We need 48 hours to process a prescription refill.

No Shows and Late Shows
If you will not be able to keep an appointment, please call our office to cancel as far in advance as possible so that we can see another patient in that time slot. We will try to give you a reminder call one to two days prior to scheduled visits to let you know that an appointment is coming up. If you are more than 15 minutes late for an appointment, we may have to reschedule you on a future date. If you miss an appointment without canceling, we may charge you a missed appointment fee.

Children’s Clinic Forms
Some of the forms we routinely use are available for you to print and fill out at home.
