Your child MUST attend a WIC appointment with a PARENT or LEGAL GUARDIAN

 

Documentation you need to BRING WITH YOU to your appointment:

 

Pregnant Women:

(You NEED to BRING)

      * Your Due Date (EDC)      

      * Current weigh/height

      * Current blood pressure    

      * Prenatal Hemoglobin

           or Hematocrit  

 

 

Postpartum or Breastfeeding Women:

(You NEED to BRING):

      * Current height/weight             

      * Hemoglobin or hematocrit

           done after delivery

           - we will accept the hgb.

           from hospital discharge papers 

For Children: (You NEED to Bring):

      * Immunization Records    

      * Current height/weight

           less than 30 days for infant

           less than 60 days for a child 1-5

      * Current hemoglobin/hematocrit—less than 90 days from WIC appointment

If your infant is on any FORMULA other than EMFAMIL or PROSOBEE, a signed statement signed and dated by your pediatrician is needed . It must include the name of the formula, the medical reason for using this formula instead of Enfamil or Prosobee, and the length of time your baby needs this formula.

If you are eligible for WIC, you will receive checks the

DAY of YOUR APPOINTMENT.

You may then be given WIC appointments at a WIC Clinic

closer to your home.
