If your infant is on any FORMULA other than EMFAMIL or PROSOBEE, a signed statement
signed and dated by your pediatrician is needed . It must include the name of
the formula, the medical reason for using this formula instead of Enfamil or
Prosobee, and the length of time your baby needs this formula. |

If you are eligible for WIC, you will
receive checks the DAY of YOUR APPOINTMENT. You may then be given WIC appointments at a
WIC Clinic closer to your home. |
