We accept private insurance, military insurances, Medicaid, Child Health Plus and offer a reduced fee to families without insurance.
We are a Patient Centered Medical Home.
That means we coordinate care to achieve our vision “Healthy Bodies, Healthy Minds, Healthy Smiles”. Being “patient-centered” means we work with you as a partner in your child’s care in a practice that is accessible, continuous, comprehensive, coordinated and compassionate.

Your First Visit:.
The patient must be accompanied to their first visit by a parent or guardian.
Having a signed Health Care Proxy Form will allow someone else to bring your chil to future visits.
Please bring with you:
Your child’s social security card
A valid insurance card
Your co-pay (if your insurance has co-pays)
Also, please bring your child's immunization record to any well check appointment.
If you have new insurance and have not yet received your insurance card, you will need the following
information: insurance company name, claim address, co-payment amount, and policy holder name, policy number and group number.
First visits require some paperwork. You can download most of the forms ahead of time and fill them
out before you come. See our FORMS section

24-hour on call service: Whenever the office is closed, one of our providers can always be reached
by calling the office number (315) 782-6400.
Call at any hour if your child is truly ill, but please leave routine calls for regular office hours.
