| Welcome! Thank you for trusting us with your family’s dental care. North Country Children’s Clinic is a private, not-for-profit agency. We offer dental care to both children and adults, no matter what their income. We give care which treats decay and helps keep your teeth and mouth healthy. If you need specialized treatment such as tooth replacement, root canals, braces, crowns, dentures, partial bridges or periodontal treatment, we will refer you to another dental provider. We accept private insurance, military insurances, Medicaid, Child Health Plus and offer a reduced fee to families without insurance. Becoming our Patient:. Your First Visit:. You will need to complete a New Patient Packet before you can be scheduled for an appointment. The patient's social security card A valid insurance card Your co-pay (if this applies)
If the patient is a child who is under 18, he or she must be accompanied to their first visit by a parent or guardian. Having a signed Health Care Proxy Form will allow someone else to bring your child to future visits. However an adult must still be with them for every office visit.
